Job Title: Centralized Admissions Coordinator

We are seeking a highly organized and detail-oriented Centralized Admissions Coordinator to join our team at North Suffolk Community Services Inc.

Key Responsibilities:
  • Ensure timely responses to the Centralized Admissions line and monitor data regularly.
  • Manage client records and charts to ensure they meet agency standards.
  • File and scan client intake information into medical records.
  • Process and mail or fax PCP releases and forms.
  • Document correspondence with outside sources via Information Exchange Note.
  • Enter SDOH screener in eHana and required paperwork.
  • Monitor patient notification system and BHHL referrals and responses.
  • Run client insurance in real-time.
  • Coordinate with collateral providers and community partners.
  • Complete all documentation in the electronic health record.
  • Facilitate communication to treatment teams when patients are admitted.
  • Review insurance and payment options with clients as needed.
Requirements:
  • Education: High School Diploma or GED completion required, Associate Degree preferred.
  • Skills: Previous office experience and computer skills required, knowledge of Windows applications (Word, Excel, and Access).
  • Experience: 1-2 years of human service experience with similar population preferred.
