About the Role

We are seeking a highly skilled and compassionate Mental Health Clinician to join our team at Scismic Inc. as part of our Safe Options Support (SOS) initiative. The successful candidate will play a pivotal role in providing comprehensive care to street homeless or subway dwelling individuals in New York City.

Job Summary

The Mental Health Clinician will be responsible for community outreach, engagement, and support of participants in the SOS program. This will involve working with a multi-disciplinary team to provide evidence-based care, including Critical Time Intervention, to help individuals achieve stability, independence, and well-being in their new community.

Key Responsibilities
  • Persistent and assertive outreach and engagement with participants using strength-based approaches.
  • Collaboration with current street outreach teams, local police precincts, hospitals, and other stakeholders to identify those in need of outreach and care.
  • Assessment of participants' health and social needs through conversational and observational assessments.
  • Work with the centralized SOR Hub to identify available housing and support participants through the process.
  • Participation in hospital discharge planning meetings to identify community resources for returning patients.
  • Providing short-term therapeutic counseling and support to participants pre and post-housing.
  • Supervision of case managers and peers on the SOS team.
  • Data collection and reporting, as required.
Requirements
  • Master's degree or higher in social work, mental health counseling, nursing, or psychology.
  • Licensed social worker, mental health counselor, or therapist in New York State.
