Job Summary

An exciting opportunity for a skilled mental health professional to join our Assertive Community Treatment (ACT) team at ACMH, Inc. as a Mental Health Clinician. In this role, you will work collaboratively with a multidisciplinary team to provide comprehensive care to individuals with complex mental health needs.

Key Responsibilities
  • Develop and implement individualized treatment plans to address clients' mental health needs.
  • Provide individual, group, and family therapy to help clients process trauma and adopt healthy coping strategies.
  • Collaborate with the ACT team to identify clients' needs and develop a comprehensive care plan.
  • Conduct home visits to assess clients' living conditions and provide support as needed.
  • Work with clients to identify and achieve their goals, including stable housing, social connectivity, and sobriety.
  • Participate in team meetings and clinical conferences to ensure seamless care coordination.
  • Document all interventions and client interactions in the electronic health record.
Requirements

To be successful in this role, you will need:

  • A Master's degree in social work, mental health counseling, nursing, or psychology.
  • At least one year of experience providing direct service in the human service field or nursing.
  • Competency in clinical documentation, motivational interviewing, trauma-informed care, and cognitive-behavioral strategies.
  • Strong written and verbal communication skills.
  • LMSW, LCSW, or LHMC or limited permit required.
