Position Overview
As a vital member of the healthcare team, the Outpatient Pharmacy Clinical Pharmacist is responsible for the preparation, compounding, and dispensing of medications to patients in accordance with applicable laws and regulations. This role emphasizes the optimization of pharmacotherapy through the identification and resolution of medication-related issues, ultimately enhancing patient outcomes.

Key Responsibilities
1. Leadership
a. Provide guidance and support to pharmacy staff, ensuring adherence to policies and promoting professional conduct.
b. Offer constructive feedback for performance evaluations of pharmacy personnel and other healthcare providers as necessary.

2. Staffing and Competency
a. Coordinate the efficient delivery of pharmacy services.
b. Ensure compliance with all relevant laws and regulations in the preparation and dispensing of medications.
c. Evaluate medication orders for accuracy and appropriateness.
d. Supervise the work of support staff to ensure quality and safety.
e. Provide drug information and consultations to healthcare professionals regarding medication therapy.
f. Identify and manage drug therapy-related problems, including potential contraindications and interactions.
g. Participate in collaborative drug therapy management protocols when applicable.
h. Evaluate and process outpatient prescriptions as necessary.
i. Conduct physical assessments and interpret lab results when appropriate.
j. Facilitate the acquisition of outpatient medications by addressing insurance-related issues.
k. Engage in pharmacokinetic monitoring and recommend adjustments as needed.
l. Maintain records for controlled substances and investigational drugs.
m. Collaborate with healthcare teams during rounds and provide discharge counseling.
n. Support research initiatives by ensuring compliance with study protocols.
o. Train and mentor new staff members.

3. Quality and Safety
a. Implement departmental policies and procedures effectively.
b. Adhere to regulations set forth by the Board of Pharmacy and other governing bodies.
c. Monitor and document clinical interventions and medication errors.

4. Technology
a. Utilize pharmacy-specific technology and troubleshoot issues as needed.

5. Education
a. Participate in ongoing education and departmental meetings.
b. Engage in quality improvement initiatives and contribute to staff development.

Qualifications
1. Eligibility for licensure to practice pharmacy in the relevant jurisdiction.
2. Completion of continuing education credits as required for licensure.
3. Completion of safety training and competency assessments as mandated.

Skills and Abilities
1. Strong communication skills for effective interaction with healthcare team members.
2. Ability to supervise and motivate pharmacy staff.
3. Strong organizational skills and ability to work under pressure.
