Role Summary

As a community health center, CSC is committed to providing empathetic, multilingual, and culturally attuned care to the residents of Chinatown, San Gabriel Valley, and surrounding areas. The Advanced Practice Nurse will operate under the guidance of the Chief Medical Officer, delivering high-quality patient care by applying their medical expertise and clinical skills.

Key Responsibilities

A. Patient Care

  • Review and assess patient records, including allergies, medical history, and immunization status.
  • Gather and document comprehensive information regarding the patient's health history.
  • Conduct physical examinations to identify symptoms or health concerns.
  • Order and interpret diagnostic tests to evaluate patient conditions.
  • Administer or prescribe appropriate treatments and medications.
  • Discuss potential side effects of medications with patients and their families.
  • Provide vaccinations to protect patients from infectious diseases.
  • Promote health awareness by advising patients on nutrition, hygiene, and preventive measures.
  • Conduct thorough patient visits, including assessments and care plans.
  • Follow up on patients' progress, especially in high-risk situations.
  • Perform physical examinations for school admissions or job eligibility.
  • Dispense medications as required.

B. Quality Management and Program Evaluation

  • Document clinical reports in accordance with legal and medical standards.
  • Assist in creating educational materials for patient outreach.
  • Evaluate and suggest improvements to medical procedures and protocols.
  • Provide training and support to staff on clinical practices.
  • Ensure quality control measures are maintained and updated.
  • Participate in regular staff and committee meetings.
  • Maintain necessary continuing education requirements for licensure.
  • Engage in workshops and training sessions as necessary.
  • Participate in outreach initiatives and community events as needed.
  • Deliver health education to patients and the community.

C. Compliance and Safety

  • Safeguard the confidentiality of all patient and employee information.
  • Adhere to regulations concerning corporate integrity and report any unethical behavior.
  • Stay informed about policies related to safe work practices.
  • Follow safety protocols and report any hazardous conditions.
  • Utilize proper body mechanics to prevent workplace injuries.
  • Be aware of emergency procedures and equipment locations.
  • Comply with infection control protocols.
  • Respond compassionately to patients, respecting their cultural and personal beliefs.
  • Maintain professional relationships with colleagues and clients.
  • Collaborate effectively to achieve departmental goals.

D. Additional Responsibilities

Perform other duties as assigned by management.

Qualifications

  • Licensed as an Advanced Practice Nurse in California.
  • Demonstrated experience in Primary Care, Pediatrics, or Urgent Care.
  • Current Furnishing number.
  • BLS/CPR Certification required.
  • DEA – X Waived preferred.
  • Commitment to ongoing professional development through relevant continuing education.
  • Bilingual proficiency in English and Spanish or Mandarin/Cantonese preferred.
  • Experience in Pediatrics is advantageous.
