The Pulmonary Rehabilitation Department focuses on adult patients suffering from chronic pulmonary conditions. This includes individuals recovering from lung surgeries and those with varying degrees of chronic lung diseases. Referrals for this program must come from the physician managing the patient's respiratory condition. The team consists of a Medical Director, Clinical Nurse Specialist, Clinical Exercise Physiologists, and a Patient Service Representative.
We are excited to announce an opportunity for a Clinical Nurse Specialist within the Pulmonary Rehabilitation Department. This advanced practice role involves leading the design, implementation, and evaluation of tailored care programs at both the unit and system levels.
Key Responsibilities:
- Influence and impact care through three spheres: client, personnel, and organizational systems.
- Generate and assess evidence-based practices.
- Collaborate with the healthcare team to ensure high-quality care for patients with pulmonary conditions.
- Evaluate patient eligibility for participation in the program through comprehensive assessments.
- Develop and execute individualized treatment plans for patients.
- Assess patients' educational needs regarding self-care strategies, including exercises, dietary modifications, and proper use of medical devices.
- Provide education on effective recovery strategies and lifestyle changes.
- Document patient care services accurately to ensure safety and comfort.
- Monitor and evaluate patient symptoms and reactions to treatments, initiating emergency procedures as necessary.
- Adapt care to meet the diverse needs of patients, considering cultural, spiritual, and psychosocial factors.
Qualifications:
Educational Background:
- Graduate from an accredited nursing program.
- Master of Science in Nursing (MSN) required.
Licensure and Certification:
- Active California State RN License required.
- Certification in Advanced Cardiac Life Support from American Red Cross or American Heart Association.
- Board certification as a Clinical Nurse Specialist.
Experience:
- A minimum of three years of recent acute care RN experience in a specialty area.
- Preferred experience in a supervisory or leadership role with strong organizational skills.
- Familiarity with relevant Standards of Practice.
- Commitment to customer service and meeting the needs of patients and healthcare colleagues.
- Meets clinical criteria for the designated level of Clinical Nurse Specialist.
