Job Summary

As a Home Health Registered Nurse with CenterWell Home Health, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our team is dedicated to providing personalized, comprehensive care to patients managing chronic conditions or recovering from injury, illness, or surgery.

Key Responsibilities
  • Provide skilled nursing visits to home health patients, including admission, case management, and follow-up care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Confer with physicians to develop and revise care plans based on ongoing assessments and patient progress.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Monitor patient progress, report to physicians and Clinical Managers, and make adjustments to care plans as needed.
  • Supervise and oversee the work of home health aides and personal care workers, ensuring high-quality care and documentation.
  • Prepare and complete necessary clinical documentation, including visit reports, patient records, and care plan updates.
Requirements
  • Diploma, Associate, or Bachelor's Degree in Nursing
  • Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
