Home Health Nurse

We are seeking a skilled and compassionate Home Health Nurse to join our team at CenterWell Home Health. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide comprehensive patient assessments and develop individualized care plans to meet the unique needs of each patient.
- Administer skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
- Conduct regular patient visits to assess progress, provide education, and make adjustments to care plans as needed.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum of one year of nursing experience, preferably in home health or a related field.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills, with the ability to work independently and as part of a team.
At CenterWell Home Health, we offer a competitive compensation package, including a salary range of $69,800 - $96,200 per year, depending on experience. We also provide a comprehensive benefits program, including medical, dental, and vision coverage, 401(k) retirement savings plan, and paid time off.
