LPN Community/Residential Care Specialist

We are seeking a skilled LPN to join our team as a Community/Residential Care Specialist. In this role, you will provide high-quality nursing care to clients in a community/residential setting.
Essential Job Functions- Administer and oversee self-administered and prescribed medications per established schedules as ordered.
- Monitor clients' vital signs and other physical health information, maintaining accurate records reflecting the data.
- Administer injectable medications, ensuring accurate prescribed dosage and timely delivery, educating clients in management of side effects.
- Complete and maintain accurate documentation relating to provision of care, medications, and other client-related services.
- Maintain client medical record/charts relating to admissions and discharges.
- Transcribe medical provider orders and obtain requested medical information pertaining to clients.
- Administer pregnancy tests and drug screens on clients and document results as needed.
- Maintain accurate inventory counts of all medicines and medical supplies.
- Schedule outside medical appointments for clients, as ordered by Medical provider.
- Monitor medical equipment and testing per CLIA standards.
- Up to eighteen months of education or training beyond the high school level in nursing or related field of study.
- License for Practical Nursing in Commonwealth of Kentucky.
- Graduation of an Accredited program.
- Minimum 6-12 months nursing experience.
The position has some unusual physical demands, including occasional exposure to threat of bodily harm from clients and their families.
Normal tasks may involve routine but limited exposure to blood, body fluids or tissue.
Reliable transportation for frequent travel between work sites.
