Oncology Care Coordinator

Rush is seeking an Oncology Nurse Navigator 2 to promote the effective utilization of services and coordination of care for patients within the Cancer Center service line. This role requires a registered nurse (RN) who can work with physician practices and multi-disciplinary teams to ensure high quality care while supporting quality metrics of the Cancer Center and Rush University Medical Center.
The ideal candidate will have at least one year of nursing experience, earned a baccalaureate level of education or higher, and possess oncology nursing certification. Strong communication and problem-solving skills are essential for this position, as the RN will serve as a liaison between patients, physician practices, the hospital, and the community. The salary for this role is approximately $80,000 - $100,000 per year, depending on experience.
Responsibilities
The Oncology Nurse Navigator 2 will be responsible for establishing a caring therapeutic relationship with patients and families, utilizing effective communication while managing a defined patient caseload. They will assess patients for social determinants of health, develop a plan of care, and mutual goal setting. The RN will also serve as a reference regarding clinical trials, coordinate care with the clinical trial office, provide psycho-social support, and facilitate referrals and transitions of care ensuring effective utilization of internal and community resources.
This role requires a strong understanding of evidence-based practice, technical expertise, critical thinking, and leadership. The RN must be able to prioritize complex patient care based upon knowledge and clinical experience, recognize changes in patient status, and respond in a timely manner to optimize patient outcomes. Additionally, they will demonstrate accountability for professional practice, continuing education, and progress toward achievement of goals.
Requirements
To be considered for this role, applicants must have current State of Illinois Registered Nurse licensure, at least one year of nursing experience, earned a baccalaureate level of education or higher, and oncology nursing certification preferred. Physical demands include the ability to travel throughout the Medical Center and some roles may require travel to other sites of care.
