RN Hospice Care Coordinator

Capital Hospice & Palliative Care is seeking a dedicated RN to lead our hospice care team. As a Hospice RN Case Manager, you will coordinate care for patients with complex medical needs, ensuring high-quality services and maintaining compliance with regulatory requirements.
Your responsibilities will include:
- Conducting initial and ongoing comprehensive assessments to determine patient care needs
- Developing individualized care plans in collaboration with the IDG, PCP, patient & families
- Providing education to patients and families on hospice services and philosophy
- Coordinating with community health representatives and other healthcare professionals to ensure comprehensive care
- Maintaining accurate records of patient care and treatment
Qualifications:
- Graduate of an accredited school of nursing
- Valid license as a Registered Nurse in the State of Illinois
- Valid IL driver's license and auto insurance
- Hospice experience (2 years) preferred
Estimated Salary: $90,000 - $115,000 per year based on location and experience.
