Key Responsibilities- Participate as a member of the emergency department care team, providing social services expertise in response to care team referrals and case finding of high-need populations.
- Conduct psychosocial assessments of patients in the emergency department, identifying needs, developing, and implementing plans of care, and documenting according to departmental standards.
- Assist patients and their support systems with coping strategies related to illness, and provide information and referrals to community resources for post-discharge support.
- Apply knowledge base, theoretical concepts, and essential social values to effect change in human behavior, emotional responses, and social conditions.
- Contribute to the patient discharge planning process, assessing patient and support system choices, and educating them on the risks and benefits of their choices to facilitate discharge.
- Provide information, resource identification, and referral services, mediation services, advocacy services, and education to patients and their support systems.
- Minimum one year of experience in a healthcare setting.
- Masters degree in Social Work.
- Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
- Basic Life Support certification from the American Heart Association.
