Job Summary

We are seeking a skilled Nurse Care Coordinator to join our team at Kalihi-Palama Health Center. As a key member of our Patient Centered Health Care Home team, you will play a vital role in providing patient-centered care and improving health outcomes.

Key Responsibilities
  • Lead the planning, implementation, and coordination of care for patients with complex and chronic illnesses.
  • Assess patient needs and facilitate referrals and care coordination with internal and external stakeholders.
  • Develop and implement nursing care plans in accordance with provider treatment plans.
  • Identify gaps in patient care and ensure they are addressed in alignment with NCQA-PCMH, HEDIS, and Health Plan requirements.
  • Implement effective interventions to reduce inappropriate ER visits and length of hospital stay.
  • Provide staff training on care coordination and chronic disease management as needed.
  • Organize and coordinate chronic disease patient classes, such as diabetes, asthma, and CKD.
  • Work collaboratively with other teams to achieve the goals and objectives of the Patient Centered Health Care Home.
  • Implement policies, procedures, and processes developed by the PCHCH team.
  • Ensure that KPHC's PCHCH team asserts changes based on the components of the chronic care model, KPHC guidelines & PCHCH model and plans for improvements are implemented.
  • Provide oversight to support staff assigned to assist in care coordination activities.
  • Prepare monthly reports and updates for the senior management team and executive team as necessary.
  • Serve as a liaison for care coordination to insurance companies and other organizations.
  • Explore community resources and establish partnerships to facilitate improvements.
  • Support other multi-cultural projects to enhance patient care.
  • Perform direct nursing care, such as triage, nurse advice, and floor duties, as needed.
Requirements
  • Licensed LPN in the State of Hawaii required.
  • Licensed RN in the State of Hawaii preferred.
  • Bachelor of Science in Nursing preferred.
  • 2 years of general experience providing patient care in a community or hospital setting.
  • 1 year of case management experience or experience providing health education and outreach activities.
