Role Overview:

The Alignment Jump Start Assessment (JSA) program is a physician-led initiative designed to support patients with complex and chronic care needs. As a JSA Advanced Practice Clinician (APC), you will collaborate with an interdisciplinary team, including physicians, nurse practitioners, physician assistants, and case managers, to deliver high-quality clinical and home-based patient care.

Key Responsibilities:

  • Conduct in-home assessments and some outpatient clinical work for health plan members
  • Perform comprehensive history-taking, physical examinations, medication reviews, and necessary cognitive/depression/safety screenings
  • Engage in end-of-life care discussions as needed
  • Collect laboratory specimens in home settings when appropriate
  • Identify diagnoses for active medical management and chronic disease management
  • Communicate assessment findings to inform primary care providers of potential care gaps
  • Educate patients, families, and caregivers, emphasizing close monitoring and follow-up
  • Utilize community resources for referrals as necessary
  • Adhere to HIPAA regulations and maintain the security of protected health information (PHI)

Qualifications:

  • Master's degree from an accredited Nurse Practitioner or Physician Assistant program
  • Preferred one year of clinical or home care experience
  • Previous experience with electronic medical records (EMR) is preferred
  • Active California state Nurse Practitioner or Physician Assistant license
  • Active RN License and Furnishing number (for Nurse Practitioners)
  • Current Nurse Practitioner Board Certification or Physician Assistant Certification
  • NPI Number, DEA, and valid BLS certification
  • Valid California driver's license and current automobile insurance
  • Experience in geriatric patient care is preferred
  • Strong administrative, organizational, and communication skills are essential

Physical Requirements:

The physical demands of this role include regular communication, standing, walking, sitting, and the ability to lift and move up to 20 pounds. Specific vision abilities required include close vision and the ability to adjust focus.

Compensation:

Pay range for this position is $75 - $90 per hour.
