Position DetailsRole: Primary Care Nurse Practitioner
Work Schedule: Full-Time, Monday – Friday, 8:00 AM – 4:30 PM (No Weekends)

Employment Type: Full-Time
As a Primary Care Nurse Practitioner, you will be instrumental in the management of both acute and chronic health conditions for our elderly participants. Your collaboration with an interdisciplinary team will enhance the quality of care provided to our community.
- Provide thorough primary care, addressing both acute and chronic health issues.
- Facilitate referrals to in-house specialists, including mental health services.
- Analyze and manage diagnostic imaging and laboratory results for precise treatment.
- Prescribe medications and formulate treatment strategies for various health conditions.
- Maintain accurate documentation of all patient interactions in the electronic medical record system.
- Conduct Skilled Nursing Facility (SNF) assessments as required.
- Engage actively in PACE Interdisciplinary Team (IDT) meetings.
- Educate participants and their caregivers on health management and preventive care.
- Comply with PACE regulations and organizational policies, ensuring credentials and certifications are current.
- Education: Graduate of an accredited Nurse Practitioner or Physician Assistant program. Must hold a valid California license and be board-certified.
- Experience: New graduates are welcome to apply. While a minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant is preferred, candidates with relevant training in community health settings will also be considered.
- In-depth knowledge of primary care practices for older adults.
- Ability to effectively engage with diverse populations.
- Proficient in electronic medical record systems and basic computer skills.
- Strong organizational and communication abilities.
- Leadership experience is a plus.
- Bilingual skills (English/Spanish) are highly valued.
