Job Summary

Acentra Health is seeking a skilled Clinical Evaluator to join our team. As a Clinical Evaluator, you will be responsible for conducting face-to-face assessments of individuals with serious mental illness, intellectual disability, or related conditions. Your goal will be to determine if these individuals meet the criteria for SMI or ID/RC and assess their level of care needs.

Key Responsibilities
  • Conduct thorough and accurate clinical assessments of patients, including relevant records and collateral sources.
  • Recommend the appropriate level of care and service needs based on patient's physical, behavioral, and psychological functioning.
  • Ensure timely completion of assessments and compliance with program workflows.
  • Develop and maintain strong relationships with patients, families, and healthcare teams.
  • Stay up-to-date with the latest clinical practices, policies, and procedures.
Requirements
  • Active unrestricted licensure in North Carolina in a healthcare field such as a Licensed Psychologist, Licensed Clinical Social Worker, Licensed Clinical Mental Health Counselor, Licensed Psychological Associate, or RN (with master's degree in Psychiatric Nursing or relevant field).
  • Graduation from an accredited College with relevant Degree Program.
  • A clean MVR (Motor Vehicle Record) and reliable transportation to travel in the state of North Carolina.
  • 3+ years of practical experience in working with mental illness.
  • 3+ years of experience in working with individuals with intellectual disability, developmental disabilities, or related conditions.
