Premise Health Systems is seeking a skilled Clinical Care Specialist to join our team in Greenville, NC. As a key member of our healthcare team, you will play a vital role in delivering exceptional patient care and support.

Key Responsibilities:

  • Conduct thorough patient interviews to determine medical needs and document findings in patient charts.
  • Obtain and record patient vital signs and weight, ensuring accurate and timely documentation.
  • Assist physicians with patient examinations as needed, providing support and expertise to ensure seamless care.
  • Review physician orders, lab requests, and follow-up needs with patients, ensuring clear communication and coordination.
  • Document services performed on appropriate forms, maintaining accurate and up-to-date patient records.
  • Apply dressings, compresses, ice, and heat packs as instructed, promoting patient comfort and well-being.
  • Observe patients and report adverse reactions to medication or treatment to medical personnel, ensuring prompt attention and care.
  • Administer specified medication, orally or by subcutaneous or intermuscular injection, and note time and amount on patient charts.
  • Assemble and use clinic equipment, maintaining a safe and efficient work environment.
  • Collect samples, such as urine, blood, and sputum, from patients for testing and perform routine laboratory tests on samples.
  • Sterilize equipment and supplies, adhering to strict infection control protocols.
  • Clean rooms and change linens, maintaining a clean and comfortable environment for patients.
  • Maintain supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.
  • Respond to patient or client telephone inquiries, evaluating and responding to patient concerns and questions.
  • Follow up with patients who miss appointments, ensuring timely and effective communication.
  • Follow up with patients on lab results, including scheduling additional tests as needed.
  • Coordinate referrals for patients through insurance and other physician offices, streamlining care and communication.

Requirements:

  • Graduate of a National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from a two-year college or technical school.
  • Licensed as a Practical or Vocational Nurse.
  • Current hands-on Certification in Basic Life Support is required, Advanced Cardiac Life Support may also be required based on scope of contract services.
  • At least three years of clinical experience as a LPN/LVN in an ambulatory care, occupational health, or emergency department setting.
