Job Summary

We are seeking a skilled LPN to join our team on the Long Term Care Unit and Memory Care Unit.

The successful candidate will provide nursing care and assistance with daily living, as well as programs and activities tailored to residents' abilities and needs.

Responsibilities
  • Provide nursing care and assistance with daily living to residents on the Long Term Care Unit and Memory Care Unit.
  • Develop and implement individualized care plans to meet residents' needs.
  • Collaborate with interdisciplinary teams to ensure high-quality care and services.
  • Participate in ongoing education and training to enhance skills and knowledge.
Requirements
  • Graduate of an accredited school of nursing.
  • Current LPN license or compact license recognized by the State of Vermont required.
  • Must possess a high aptitude for learning, strong customer service skills, and teamwork skills.
Preferred Qualifications
  • Previous long term care experience.
  • Basic computer skills.
