Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team at The University of Vermont Health Network. As an LPN, you will play a vital role in providing high-quality patient care and support to our residents in a skilled nursing facility setting.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination
  • Communicate effectively with residents, families, and healthcare teams to ensure informed decision-making
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
  • Graduate of an accredited LPN program
  • Current LPN license or compact license recognized by the State of Vermont
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced, dynamic environment
  • Basic computer skills and proficiency in electronic medical records
Preferred Qualifications
  • Previous long-term care experience
  • Experience working with geriatric populations
  • Basic life support certification
