About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Post Acute Care team at Helen Porter Rehabilitation & Nursing. As an LPN, you will play a vital role in providing high-quality patient care and support to our residents.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Graduate of an accredited LPN program.
  • Current LPN license or compact license recognized by the State of Vermont.
  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Basic computer skills and proficiency in electronic medical records.
