About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Porter Medical Center. As an LPN, you will play a vital role in providing high-quality patient care and support to our residents in our Long Term Care and Memory Care Units.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Assist with daily living activities, such as bathing, dressing, and grooming.
  • Participate in interdisciplinary team meetings to develop and implement individualized care plans.
  • Collaborate with other healthcare professionals to ensure seamless care transitions.
  • Document patient information, including medical history, treatment plans, and progress notes.
Requirements
  • Graduate of an accredited LPN program.
  • Current LPN license or compact license recognized by the State of Vermont.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Basic computer skills and proficiency in electronic medical records.
