Key Responsibilities:
  • Conduct thorough assessments to identify individual needs and develop comprehensive care plans to address objectives and goals.
  • Support members in accessing quality and cost-effective care, modifying plans as needed to ensure optimal outcomes.
  • Coordinate with internal and external resources to meet the needs of our members, incorporating social determinants of health into care planning.
  • Build meaningful partnerships with designated populations and their families, addressing cultural and linguistic needs to promote better health outcomes.
  • Maintain knowledge of the system of care philosophy and work closely with various state agencies to ensure seamless care coordination.
Requirements:
  • Master's degree in social work, counseling, or a related behavioral health field, or a degree in nursing with a minimum of 3 years of clinical experience in social work counseling.
  • Current active unrestricted license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States.
Preferred Skills:
  • Experience working with specialty populations.
  • Clinical experience in social work counseling with a broad range of complex psychiatric/substance abuse and/or medical disorders.
