Job Summary:

This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The Case Manager I/II will obtain in-depth social histories, develop and assure community and/or agency resources to meet the consumer's needs, and serve as a Qualified Intellectual Disabilities Professional (QIDP) providing monthly monitoring of the program.

Responsibilities:

Obtain in-depth social histories of individuals with intellectual and/or developmental disabilities
Develop and assure community and/or agency resources to meet the consumer's needs
Serve as a Qualified Intellectual Disabilities Professional (QIDP) and provide monthly monitoring of the program

Requirements:

Bachelor's degree in a human service field or a valid permanent Oklahoma Nursing license
One year of general experience working directly with individuals with developmental and/or intellectual disabilities or one year of professional experience working directly with individuals with developmental and/or intellectual disabilities in a related field

Working Conditions:

This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. Travel is extensive, and the position requires a valid driver's license and required car insurance.
