Behavioral Health Case Manager

The Behavioral Health Case Manager I is a vital role at Elevance Health, responsible for providing comprehensive care to members with behavioral health and substance abuse needs. This position requires a strong clinical background, excellent communication skills, and the ability to work effectively in a hybrid model.
Key Responsibilities:
- Conduct thorough assessments to identify individual needs and develop personalized care plans.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
- Collaborate with internal and external resources to coordinate care and support members in achieving their goals.
- Support members in accessing quality and cost-effective care, while promoting healthy behaviors and lifestyle choices.
Requirements:
This role requires a Master's degree in a related field, such as social work, counseling, or nursing, along with a minimum of 3 years of clinical experience in behavioral health. A current, active license to practice as a health professional within the scope of licensure in applicable states or territory of the United States is also required.
Preferred Skills:
- Experience in case management and telephonic coaching with members who have complex psychiatric and substance abuse disorders.
- Knowledge of screening criteria and clinical judgment to assess member needs and develop effective care plans.
