Healthcare Case Manager

Job Summary:

This is a full-time telework role in Kansas, with 50-75% travel for face-to-face visits in assigned areas. The schedule is Monday-Friday, standard business hours. The successful candidate will develop, implement, support, and promote Health Services strategies, tactics, policies, and programs that drive the delivery of quality healthcare to establish competitive business advantage for Healthcare.

Key Responsibilities:

  • Conduct comprehensive evaluation of referred members' needs/eligibility and recommend an approach to case resolution and/or meeting needs by evaluating members' benefit plans and available internal and external programs/services.
  • Identify high-risk factors and service needs that may impact member outcomes and care planning components with appropriate referral to clinical case management or crisis intervention as needed.
  • Coordinate and implement assigned care plan activities and monitor care plan progress.

Requirements:

  • A degree in a behavioral health, social services, or human services field.
  • Experience in case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.
