About the Role:

The Foster Care Coordinator is a critical position at UnitedHealthcare, responsible for providing comprehensive care coordination services to children and youth in foster care. This role requires a strong understanding of trauma-informed care and the ability to work with complex, high-needs populations.

Key Responsibilities:

  • Assess and develop individualized care plans for children and youth in foster care, taking into account their unique needs and circumstances.
  • Coordinate with community resources, including healthcare providers, social services, and educational institutions, to ensure seamless care delivery.
  • Provide education and support to caregivers and families, empowering them to make informed decisions about their child's care.
  • Advocate for children and youth in foster care, ensuring their voices are heard and their needs are met.
  • Collaborate with the child welfare and juvenile justice systems to ensure coordinated care and support.

Requirements:

  • Masters degree in social work, nursing, psychology, or related field.
  • 2+ years of experience working in a community health setting, preferably with children and youth in foster care.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to travel in assigned region to visit Medicaid members in foster homes and other settings.

Preferred Qualifications:

  • Licensed Social Worker or clinical degree.
  • Background in managing populations with complex medical or behavioral needs.
  • Experience with electronic charting and community resource coordination.
