Job Summary

We are seeking a highly motivated and compassionate Community Health Worker (CHW) to join our team in New Mexico. As a CHW, you will play a vital role in increasing access to ABA services, driving better care outcomes, and breaking down barriers hindering community members from accessing these essential resources.

Key Responsibilities
  • Engage with clients on our waitlist, including referral coordination, patient education, client and family onboarding, and assessment coordination.
  • Engage clients in ongoing services, including client and family orientation, client scheduling, and supporting clients and families who have paused or are on hold services.
  • Operate within local, state, and federal regulations, as well as company policies and procedures.
  • Collaborate closely with pediatric clients, caregivers, and our care team to ensure seamless service delivery.
Requirements
  • A Community Health Worker certification from the NMDOH Office of Community Health Workers.
  • Excellent communication and interpersonal skills with a dedication to family-centered care.
  • Position is eligible for 75% remote work from home, with 25% travel to our Frontera Health New Mexico regions.
  • MSW or LCSW candidates are strongly encouraged to apply, although these are not required qualifications.
