About the Role

Frontera Health, Inc. is seeking a dedicated Community Health Worker to join our team in New Mexico. As a Community Health Worker, you will play a vital role in increasing access to ABA services and driving better care outcomes for pediatric clients and their families.

Our ideal candidate is passionate about community health and has excellent communication and interpersonal skills. You will work closely with our care team to engage clients on our waitlist, provide patient education, and coordinate assessments. You will also engage clients in ongoing services, including client and family orientation, scheduling, and supporting clients and families who have put services "on pause" or who are on our "hold list".

In this role, you will operate within the New Mexico Department of Health Community Health Worker Scope of Work, which includes community outreach, community and cultural liaison, system navigation and care coordination and case management, home-based support, health promotion and training, community assessment and mobilization, clinical support, and translation.

Requirements

  • A Community Health Worker certification from the NMDOH Office of Community Health Workers
  • Excellent communication and interpersonal skills
  • Position is eligible for 75% remote work from home, however will require 25% travel to our Frontera Health New Mexico regions
