Job Summary

We are seeking a skilled LPN to join our Home Health team at Providence. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes. This is a full-time opportunity with a competitive salary and benefits package.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress and communicate with healthcare teams to ensure continuity of care.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited LPN/LVN school of nursing.
  • Current Oregon Licensed Practical Nurse (LPN) license.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift 25 pounds and stand for long periods.
