Job Title: LPN LVN Home Health Nurse

We are seeking a skilled and compassionate LPN LVN to join our Home Health team. As a Licensed Practical Nurse, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes.

Job Summary:

The LPN LVN will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing direct patient care. You will work collaboratively with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.

Responsibilities:
  • Assess patients' physical, emotional, and social needs
  • Develop and implement individualized care plans
  • Provide direct patient care, including wound care, medication management, and vital sign monitoring
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Document patient progress and outcomes
Requirements:
  • Graduate of an accredited LPN/LVN school of nursing
  • National Provider BLS - American Heart Association within 30 days of hire
  • Oregon Licensed Practical Nurse upon hire
  • Valid driver's license and reliable transportation
  • Ability to work independently and as part of a team
Preferred Qualifications:
  • Experience in home health, hospice, or public health
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment
