LPN LVN Home Health Job Description

We are seeking a skilled LPN LVN to join our team of healthcare professionals in providing high-quality care to patients in their homes. As a Licensed Practical Nurse, you will work alongside an interdisciplinary team to deliver whole-person care, empowering patients to achieve their optimal health and well-being.

Key Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans, taking into account patient preferences and goals.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and satisfaction.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Graduate of an accredited LPN/LVN school of nursing.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Oregon Licensed Practical Nurse upon hire.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
Preferred Qualifications:
  • Experience in home health, hospice, or public health.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
