Job Description

As a PRN/Part Time Home Risk Assessment Specialist, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed. You will also collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.

Responsibilities include:

  • Conducting comprehensive health assessments in various venues, including home visits, skilled facility visits, and telehealth visits.
  • Developing and implementing individualized care plans to address the unique needs of each patient.
  • Collaborating with healthcare teams to ensure seamless care coordination and communication.
  • Providing education and support to patients and their families to promote health and wellness.

Requirements

To be successful in this role, you will need:

  • A Master's Degree in Nursing or a related field.
  • Current RN and NP licensure in the state of practice, including prescription authority.
  • Board certification by the AANP, ANCC, or AACN in a Matrix-approved specialty.
  • A current BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with Employee Health/Medical Record software.
  • Excellent verbal and written communication skills.
  • The ability to travel and drive a car, with proof of adequate automobile insurance coverage.
