Job Title: Clinical Care Coordinator

We are seeking a highly skilled Registered Nurse to fill the role of Clinical Care Coordinator at Northwell Health. In this position, you will serve as a liaison between patients and healthcare facilities/physicians, ensuring a seamless continuum of quality patient care throughout hospitalization.

Key Responsibilities:
* Facilitate effective patient management throughout hospitalization
* Conduct concurrent utilization management using evidence-based medical necessity criteria
* Submit data to management regarding case management and quality initiatives
* Collaborate with healthcare teams to assess, plan, implement, and evaluate patient care

Qualifications:
* Graduate from an accredited School of Nursing
* Bachelor's Degree in Nursing preferred; currently enrolled in or committed to obtaining BSN within 5 years
* Minimum 2 years relevant experience, required
* Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed

The estimated salary for this position is $95,000 - $115,000 per year, based on location and experience.
