Job Title: Licensed Master Social Worker

Job Summary: NavitsPartners is seeking a skilled Licensed Master Social Worker to join our team as a Patient Care Specialist. As a key member of our healthcare team, you will be responsible for providing comprehensive social work services to patients and their families.

Key Responsibilities:

  • Patient Assessment: Conduct thorough assessments of patients' psychosocial needs, including mental health, family dynamics, and resource needs.
  • Counseling: Provide individual and group counseling to patients and their families to address emotional, social, and psychological concerns.
  • Resource Connection: Assist patients in accessing community resources, financial assistance programs, and other support services to enhance their quality of life.
  • Care Planning: Collaborate with healthcare teams to develop and implement individualized care plans that address the comprehensive needs of each patient.
  • Crisis Intervention: Respond to patient crises, providing immediate support and connecting them with emergency services as needed.
  • Documentation: Maintain accurate and timely records of all patient interactions, assessments, and care plans in compliance with regulatory and organizational standards.

Requirements:

  • Experience: At least 1 year of experience as a Licensed Master Social Worker (LMSW).
  • Licensure: Valid Licensed Master of Social Work (LMSW) license issued by the New York State Education Department (NYSED), with primary source verification.
  • Language Proficiency: Proficiency in English is required. Bilingual candidates (Spanish, Mandarin, Bengali) are preferred but not required.
  • Interpersonal Skills: Strong communication and interpersonal skills to work effectively with patients, families, and multidisciplinary teams.

Education:

  • Minimum Degree Required: Master's Degree in Social Work (MSW) from an accredited institution.
