Job Description:

As a Hospice RN/RN Case Manager, you will be responsible for providing direct patient care services, managing patient clinical records, and collaborating with an interdisciplinary team to deliver high-quality care. You will also play a key role in educating patients and their families on care options and empowering them to make informed decisions about their healthcare.

Key Responsibilities:

  • Provide direct patient care services, including assessments, care planning, and interventions
  • Manage patient clinical records, including progress notes, treatment plans, and discharge summaries
  • Collaborate with an interdisciplinary team, including physicians, nurses, social workers, and chaplains
  • Educate patients and their families on care options and empower them to make informed decisions about their healthcare
  • Participate in quality assessment and performance improvement activities

Requirements:

  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Comfortable providing care in a variety of settings, including hospitals, skilled nursing facilities, and private homes
  • Demonstrated ability to work effectively in a team environment
  • Excellent communication and interpersonal skills
