Key Responsibilities
  • Work closely with Social Workers and Nurse Care Coordinators to arrange follow-up appointments, transmit treatment plans, and coordinate patient care.
  • Assist patients in locating community resources and understanding their benefits.
  • Provide notary public services to patients and staff as needed.
  • Assist with safe transition of patients to home or other facilities.
  • Collaborate with the medical team to ensure seamless patient care and communication.
Requirements
  • Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.
  • Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.
