Position Overview:
We are seeking a skilled and innovative Licensed Practical Nurse (LPN) to join our team as a Care Coordination Specialist. In this role, you will collaborate with the clinical teams at Pure Healthcare and Tuesday Health, employing a multidisciplinary strategy to provide exceptional care for our members facing serious illnesses.

The LPN will conduct home visits and may work remotely as needed.

Key Responsibilities:
As a Care Coordination Specialist, your duties will include:

  • Administering various assessments and screening tools throughout the care timeline, supported by nurse practitioners and registered nurses.
  • Prioritizing patient needs based on assessment outcomes and coordinating with the interdisciplinary care team.
  • Being accountable for the development of care plans within the multidisciplinary team.
  • Participating in internal and joint rounds to ensure an optimal and coordinated member experience.
  • Learning and utilizing Tuesday Health's electronic systems and tools to deliver a cohesive approach, ensuring each member has a personalized experience.
  • Building strong, trusting relationships with members, where active listening and empathy are fundamental to every interaction.
  • Acting as the central point of contact for the multidisciplinary team, transforming care for members with serious illnesses.

Qualifications:
To excel in this role, you will need:

  • An active and unrestricted Licensed Practical Nurse license in the State of Ohio, free from any board actions.
  • Experience in a clinical or medical environment is preferred.
  • Experience working within a multidisciplinary care model, collaborating with network providers, health plan care management, and employed clinicians.
  • Strong communication and organizational skills.
  • The ability to multitask and perform under pressure while maintaining a positive member experience.
  • Judgment to escalate issues and act professionally in emergency situations.
  • Technical proficiency to navigate various documentation systems and tools, with a willingness to learn new systems as the organization evolves.
  • A valid driver's license with a safe driving record and state minimum automobile insurance coverage.
  • Comfort with change and ambiguity, understanding that rapid changes in business strategy and priorities are part of our dynamic environment.
