About the Role

We are seeking a skilled and compassionate Per-Diem RN to join our team at Cornerstone Hospice House. As a member of our interdisciplinary group, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Assess and identify patient/family needs, utilizing the nursing process to develop and implement individualized plans of care.
  • Coordinate with the Interdisciplinary Group to ensure comprehensive care and effective communication.
  • Provide palliative and supportive care to patients and their families, addressing physical, emotional, and spiritual needs.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.
Requirements
  • Current RN licensure in the state where the employee will be working.
  • Active BLS certification from the American Heart Association or Red Cross.
  • Minimum one year of in-patient nursing or hospice experience.
  • Previous experience working with EMR/EHR systems.
  • Valid driver's license and reliable transportation.
  • Ability to effectively communicate in English (verbal and written).
  • Flexible schedule to meet the needs of Hospice House patients.
