As a vital member of the Chapters Health System, the Registered Nurse at Cornerstone Hospice House plays a crucial role in delivering compassionate care to patients and their families. This position operates on a DAY shift schedule, allowing for a balanced work-life dynamic.
Core Responsibilities:
1. Assess and identify the needs of patients and their families, utilizing the nursing process to ensure comprehensive care.
2. Collaborate with the Interdisciplinary Group (IDG) to coordinate and implement an individualized Plan of Care.
3. Provide palliative and supportive care, addressing the psychosocial, spiritual, and physical needs of patients.
4. Educate patients, families, and caregivers on disease processes, palliative interventions, and safety practices.
5. Monitor and report changes in patient conditions to the IDG and other healthcare professionals.
6. Supervise nursing team members, ensuring high standards of care are maintained.
Qualifications:
- Current RN license in Florida or an active multistate/compact nurse license.
- Minimum of one year of experience in inpatient nursing or hospice care.
- Proficiency in using EMR/EHR systems.
- Strong communication skills in English, both verbal and written.
- Ability to manage the emotional challenges associated with caring for terminally ill patients.
Physical Requirements:
This role requires the ability to perform essential functions, including lifting, bending, and accessing patient areas. Candidates must be able to meet the physical demands of the position with or without reasonable accommodation.
