Job Summary

We are seeking a highly skilled Care Management Support Specialist to join our team at Prisma Health. As a key member of our Care Coordination Department, you will play a vital role in supporting the day-to-day operations and reporting requirements of our team.

Key Responsibilities

  • Provide administrative and technical support to the Care Coordination Department, including data entry, reporting, and record-keeping.
  • Develop a basic understanding of Care Management processes and procedures to support and intervene in areas allowable under the scope of practice for this role.
  • Collaborate with care management staff and other healthcare professionals to ensure effective communication and coordination of care.
  • Stay up-to-date with healthcare regulations and guidelines related to transitions of care and care management.

Work Environment

This is a day shift position, and you will be working in a fast-paced healthcare environment. You will have the opportunity to work with a talented team of professionals who are passionate about transforming healthcare for the benefit of our communities.
