Job Overview

Work Schedule: This is a full-time position.

Key Responsibilities:

  1. Community Psychiatric Supportive Treatment (CPST): Deliver face-to-face, community-based interventions that are solution-focused and aligned with the consumer's treatment plan.
  2. Targeted Case Management (TCM): Enhance the consumer's independent functioning by facilitating access to necessary medical, social, educational, and other services.
  3. Wraparound Facilitation: Assemble a wraparound team to create a personalized Plan of Care, ensuring all team members are engaged and informed.
  4. Additional Services: Provide or coordinate various services as outlined in the consumer's treatment plan.
  5. Documentation: Maintain accurate records of services provided in compliance with organizational policies.
  6. Training Requirements: Complete necessary training programs as mandated for service delivery.
  7. Effective Communication: Maintain open lines of communication with all stakeholders involved in the consumer's care.
  8. Other Duties: Perform additional responsibilities as assigned by supervisory staff.

Employment Status: Non-exempt

Compensation: Hourly wage

Supervision: This position reports to the Manager of Children Services.
