Clinical Team Coordinator

Job Summary
This position is part of the clinical team at Kansas Employer. The Clinical Team Coordinator will be responsible for a variety of care management activities with a focus on ensuring that all patients receive the support they need. The ideal candidate will identify areas of reeducation needs, coordinate training needs, and serve as a liaison among staff to help create continuity around the organization as related to patient quality, experience, and satisfaction.
Key Responsibilities
Supervise clinical staff, including hiring, staffing, discipline, training/education, and performance evaluation.
Observe and provide feedback on the practice of clinical staff to ensure appropriate patient care and proper implementation of required programs related to quality measures.
Serve as the primary contact for assigned clinical staff, scheduling changes, updates, and coordination of staff.
Collaborate with providers and other multidisciplinary team members to provide complete patient care.
Investigate patient and/or service complaints, involving other staff as needed.
Ensure proper usage of the electronic health record.
Provide individual professional advice and support to clinical staff as needed.
Travel to assigned sites for training and implementation of processes as needed.
Participate in the development of new and/or enhancement of existing policies, procedures, and programs to improve accuracy and quality of patient care.
Assist with developing new and/or refining existing health improvement initiatives to best meet the health needs of patients, ensuring programs utilize evidence-based practices and reach targeted populations.
Work in a direct patient care capacity on a regular basis and fill in for staffing needs.
Work independently with minimal supervision, working toward accomplishment of established goals.
