Job Summary

As a Care Coordinator LPN with Christus Health, you will play a vital role in providing high-quality patient care and coordination services. This position requires a strong understanding of healthcare protocols and the ability to work collaboratively with a multidisciplinary team to optimize patient outcomes.

Key Responsibilities:
  • Manage a caseload of 10,000-15,000 patients with identified gaps in care, working collaboratively with the healthcare team to develop and implement personalized care plans.
  • Facilitate communication between patients, caregivers, physicians, and the outpatient care team to ensure seamless coordination of services.
  • Work proactively to coordinate preventative and follow-up care for patients receiving care from participating providers, utilizing evidenced-based protocols and guidelines.
  • Review patient medical records to identify care gaps and schedule necessary preventative services, documenting encounters as required.
  • Employ knowledge of evidenced-based guidelines to effectively communicate the importance of recommended procedures and testing, promoting patient collaboration with the care team.
  • Facilitate a collaborative approach with the care team for identification and closure of care gaps, utilizing educational and behavioral change strategies to promote patient engagement.
  • Maintain confidentiality in all areas at all times, adhering to strict confidentiality and data protection protocols.
Requirements:
  • LPN License in the state of employment or compact
  • BLS certification
