  • Review patient medical records to identify areas for improvement and develop strategies to address these gaps.
  • Develop and maintain strong relationships with patients, families, and healthcare providers to ensure a collaborative approach to care coordination.
  • Employ effective communication and problem-solving skills to address complex patient needs and ensure seamless transitions of care.
  • Stay up-to-date with the latest evidence-based guidelines and best practices in patient care coordination.
Requirements
  • LPN License in the state of employment or compact
  • BLS Certification
