Position Overview

The Nurse Practitioner specializing in Palliative Care is tasked with embodying the core values of Compassion, Integrity, and Excellence while advancing the Compassus mission. The Palliative Care Nurse Practitioner delivers specialized care to patients confronting chronic, intricate, or life-limiting illnesses across various settings. Responsibilities include conducting thorough assessments, providing education on diseases and treatments, and managing symptoms to enhance the quality of life for patients and their families. This role is crucial in ensuring patients and families achieve optimal comfort and coping strategies. Collaboration with primary care providers and specialists is essential, as is compliance with established practice standards and regulations. Flexibility in scheduling is required to meet the demands of the agency.

Key Responsibilities

  • Conducts routine and urgent assessments:
    • Availability Monday – Friday, 8 AM to 5 PM local time.
    • Minimum of 100 patient visits per month, adjusted for leave and part-time status.
    • Maintains a patient load of at least 80, subject to market needs.
    • Triages consults based on patient acuity.
    • Delivers bedside palliative care along with patient and family education.
    • Participates in after-hours and weekend on-call duties as necessary.
    • Orders and interprets laboratory and imaging tests within professional scope.
    • Prescribes medications, including controlled substances, as permitted.
    • Orders necessary treatments and durable medical equipment.
    • Engages in therapeutic measures as required.
    • Consults with palliative care physicians as needed and informs primary physicians of services rendered.
    • Facilitates care coordination for palliative care referrals.
    • Maintains accurate patient records and documentation to support medical practice and service reimbursement.
    • Adheres to applicable laws and regulations regarding Collaborative Agreements.
    • Ensures accurate billing and coding for all patient interactions in accordance with CMS guidelines.
    • Provides training and educational support for staff.
    • Sets professional growth goals and actively participates in professional organizations.
    • Engages in self-care practices for personal and professional development.
    • Maintains confidentiality regarding patients, families, staff, and the organization.
    • Assists patients and families in identifying treatment goals and ongoing care plans during each visit, communicating these with the Interdisciplinary Team (IDT).
  • Collaborates with Medical Director, attending Physicians, hospital staff, and IDT:
    • Participates in monthly IDT meetings with the market team.
    • Communicates patient needs for clinical and psychosocial interventions to the IDT.
    • Identifies the need for intervention from other IDT members.
    • Effectively conveys patient and family needs to the IDT.
    • Completes and submits required clinical documentation within 24 hours of visit completion.
  • Educates patients and caregivers on:
    • Patient care practices.
    • Disease processes.
    • Goal setting strategies.
    • Symptom management techniques.
    • Treatment options available.
    • Prognosis discussions.
    • Advance Care Planning.
  • Supervises nurse trainees, including graduate and advanced practice nurses in clinical placements.
  • Participates in the supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees.
  • Performs additional duties as assigned.
Qualifications

  • Master's degree in Nursing is required.
  • A minimum of five (5) years of nursing experience is required; experience in oncology, hospice, geriatrics, or intensive care is preferred.

Skills

  • Mathematical Skills: Proficient in basic arithmetic operations and able to compute rates, ratios, and percentages.
  • Language Skills: Capable of reading and interpreting business documents, professional journals, and regulations. Proficient in writing reports and correspondence. Strong verbal and written communication skills are essential.
  • Other Skills and Abilities: Proficient in English. Embraces the philosophy of hospice care. Strong organizational and time management skills. Flexibility to work evenings and weekends as needed. Willingness to travel. Ability to maintain confidentiality of sensitive information.

Certifications, Licenses, and Registrations

  • Active and unencumbered CNS or Nurse Practitioner license in the state of employment is required.
  • Certification as a Nurse Practitioner in Adult, Family, Acute, or Geriatric care is required.
  • Active NPI number.
  • Maintain active DEA certification if state regulations permit prescription of controlled substances by Advanced Practice Providers.
  • Board Certification in Hospice and Palliative Care is preferred but not mandatory.
