Position Overview

The Advanced Practice Nurse in Palliative Care is tasked with exemplifying the core values of Compassion, Integrity, and Excellence, while fostering the Compassus philosophy grounded in the 6 Pillars of success. The Advanced Practice Nurse provides specialized palliative care for individuals confronting chronic, intricate, and/or life-limiting illnesses across various care settings. Responsibilities include conducting assessments, offering education on diseases and treatments, and managing symptoms to address the needs of patients and their families. The Advanced Practice Nurse plays a crucial role in ensuring that patients and their families achieve optimal comfort and coping strategies. Collaboration with primary care providers and specialists is essential, alongside ensuring compliance with established practice standards and regulations. Coordination with the designated Compassus Medical Director is also required, along with flexibility in scheduling to accommodate organizational needs. Adherence to the rules and regulations set forth by state and federal agencies is a critical aspect of this role.

Key Responsibilities

  • Conducts routine and urgent assessments:
    • Availability during standard business hours.
    • Minimum of 100 patient visits monthly, adjusted for leave and part-time status.
    • Maintains a patient load of at least 80, with potential increases based on market demands.
    • Ability to prioritize consults based on patient acuity.
    • Delivers bedside palliative care with education for patients and families.
    • Participates in after-hours and weekend on-call duties as required.
    • Orders, performs, and interprets laboratory and imaging tests within the scope of practice.
    • Prescribes medications, including controlled substances, as permitted by licensure.
    • Orders necessary treatments and durable medical equipment.
    • Engages in other therapeutic measures as required.
    • Consults with palliative care physicians as necessary and communicates services provided to primary physicians.
    • Facilitates care coordination for palliative care referrals.
    • Maintains accurate patient records and documentation to support medical practice and service reimbursement.
    • Complies with laws and regulations regarding Collaborative Agreements.
    • Ensures accurate billing and coding for all patient interactions in accordance with guidelines.
    • Provides training and ongoing education for staff members.
    • Sets professional growth goals and actively participates in relevant professional activities.
    • Engages in self-care practices for personal and professional development.
    • Maintains confidentiality regarding patients, families, staff, and organizational matters.
    • Assists patients and families in identifying treatment goals and ongoing care plans during each visit, ensuring communication among the Interdisciplinary Team (IDT).
  • Collaborates with Medical Director, attending Physicians, hospital staff, and IDT:
    • Participates in monthly IDT meetings with the market team.
    • Communicates IDT needs for clinical and psychosocial interventions.
    • Identifies the need for intervention from other IDT members.
    • Effectively communicates patient and family needs to the IDT.
    • Completes and submits required clinical documentation within 24 hours of visit completion.
  • Educates patients and caregivers on:
    • Patient care strategies.
    • Disease processes.
    • Goal setting techniques.
    • Symptom management.
    • Treatment alternatives.
    • Prognostic information.
    • Advance Care Planning.
  • Supervises nursing trainees, including graduate and advanced practice nurses in clinical placements.
  • Participates in the supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees.
  • Performs additional duties as assigned.
Qualifications

  • Master's degree in Nursing is required.
  • A minimum of five (5) years of nursing experience is required; experience in oncology, hospice, geriatrics, or intensive care is preferred.

Skills

  • Mathematical Skills: Proficient in basic arithmetic operations and able to compute rates, ratios, and percentages.
  • Language Skills: Capable of reading, analyzing, and interpreting business publications, professional journals, and regulatory documents. Proficient in writing reports and correspondence. Strong verbal and written communication skills are essential.
  • Other Skills and Abilities: Proficient in English. Embraces the philosophy of hospice care. Strong organizational and time management skills. Flexibility to work evenings and weekends as needed. Willingness to travel. Ability to maintain confidentiality of sensitive information.

Certifications, Licenses, and Registrations

  • Active and unencumbered CNS or Nurse Practitioner license in the state of employment is required.
  • Certification as a Nurse Practitioner in Adult, Family, Acute, or Geriatric care is required.
  • Active NPI number.
  • Maintain active DEA certification if permitted by state regulations for prescribing controlled substances.
  • Board Certification in Hospice and Palliative Care is preferred but not mandatory.
