The Case Management role is essential in providing support to individuals who have previously experienced homelessness and are diagnosed with mental health conditions. Reporting to the Senior Director of Supportive Housing, the Case Manager will utilize an "access and refer" model to deliver comprehensive case management services. This includes outreach, assessment, referral, crisis intervention, and ongoing life-skills training, all while collaborating closely with property management to ensure successful tenancy.
Key Responsibilities:
- Support a small group of homeless clients as they transition into supportive housing, ensuring they meet tenancy obligations.
- Assist a larger group of clients and families in their transition into housing, providing guidance on tenancy responsibilities.
- Manage a diverse caseload of tenants, including families, to ensure their needs are met.
- Conduct thorough assessments to gather relevant psychosocial, financial, employment, housing, educational, and health information, leading to the development of tailored Service Plans.
- Verify and maintain all necessary documentation for client support.
- Create and implement support systems to address tenant needs, coordinating various services that promote housing stability and self-sufficiency.
- Facilitate referrals to therapeutic, social service, educational, and medical resources as needed.
- Assist clients in accessing and maintaining their entitlements and benefits.
- Support tenants in developing essential daily living skills, communication abilities, and self-advocacy techniques.
- Monitor client progress in accordance with required standards.
- Participate in mandatory training sessions to uphold professional standards.
- Provide crisis intervention when necessary.
- Engage in workshops that address tenant needs.
- Contribute to outreach initiatives and community engagement events.
- Establish and nurture relationships with community resources and partners.
- Collaborate effectively with property management to enhance tenant experiences.
- Perform additional duties as assigned.
Qualifications:
- A Master's degree in social sciences or a related field, such as MSW, from an accredited institution.
- At least two years of experience in case management services is preferred.
- A strong sense of teamwork and motivation.
- Excellent communication and interpersonal skills.
- Bi-lingual or bi-cultural candidates are encouraged to apply.
- Proven ability to work with clients from diverse backgrounds.
- A commitment to the agency's mission of providing compassionate support for basic human needs.
- Proficiency in MS Word, Excel, and internet applications.
- A valid driver's license and access to a reliable vehicle.
