We are seeking a highly skilled RN Intake Coordinator to join our team at the Simmons Comprehensive Cancer Center in the department of Patient Access Services. As a key member of our team, you will play a critical role in ensuring that patient referrals are properly vetted, prioritized, scheduled, and records collected to provide a positive patient experience.
Key Responsibilities- Receive and respond to patient inquiries and referrals, establishing a strong relationship between referral sources and the hospital.
- Determine the appropriateness of referrals for admission, in accordance with established admission criteria for services involved.
- Interface with other hospital departments and physicians to facilitate the admission process, ensuring timely and efficient care.
- Assess financial reimbursement for proposed hospitalization and intervention, obtaining pre-certification as needed.
- Coordinate with patients, families, payers, and referral sources to provide an overview of services to be provided during hospitalization.
- Notify payors of admission and expected course of treatment, assigning initial length of stay in accordance with standards of care criteria.
- Track data regarding intakes and referral sources, providing insights to inform process improvements.
- Develop and implement guidelines and instructions for staff regarding program promotion, intake, and referral processes.
- Communicate information about referred patients to the rehab unit, ensuring seamless care coordination.
- Graduate of an NLN-approved nursing program.
- Three years of experience in medical intake and/or care coordination.
- Experience in relationship development and managed care.
- Current Texas (or BNE Compact State) license and Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC).
