Reports to the Manager. Conducts evaluations of patients, formulates treatment plans, and implements physical therapy interventions aimed at restoring function, preventing disability, alleviating pain, and enhancing quality of life following illness, injury, or surgical procedures.

Clinical Responsibilities
  • Conducts thorough assessments based on physician's directives and diagnoses.
  • Designs treatment plans that include time-sensitive functional objectives.
  • Regularly reassesses patients and adjusts treatment plans based on objective findings.
  • Communicates directly with physicians regarding any necessary modifications to prescribed therapy.
  • Ensures that prescriptions are current.
  • Supervises and guides therapy assistants in patient treatment planning.
  • Adheres to all regulatory guidelines for assistant supervision and patient management.
  • Initiates discharge planning at the time of evaluation.
  • Integrates skills acquired from continuing education into daily practice.
  • Employs a variety of evidence-based treatment techniques tailored to individual patient needs to optimize outcomes.
  • Educates patients and families on therapeutic activities, provides relevant materials, and documents in medical records.
  • Maintains professional and appropriate communication with patients and families according to their understanding and needs.
Operational Duties
  • Meets productivity expectations for assigned caseload.
  • Proactively offers assistance to colleagues when personal caseload is below expectations.
  • Demonstrates initiative and adaptability in managing daily caseloads.
  • Takes responsibility for daily support tasks when other personnel are unavailable.
  • Actively participates in meetings, program development, special projects, and other initiatives.
  • Meets all defined deadlines for licenses, certifications, and mandatory training.
  • Holds self accountable for understanding and adhering to policy and regulatory guidelines shared in meetings and training.
  • Consistently follows team communication norms.
  • Clearly communicates expectations to support personnel.
  • Collaborates with all relevant disciplines and ancillary staff to effectively manage patient care.
Documentation Standards
  • Complies with departmental policies regarding timely completion of all documentation (evaluations, notes, and discharge summaries).
  • Completes all documentation related to patient assessment tools used for outcomes and regulatory compliance within established guidelines.
  • Ensures documentation is detailed enough to support all billing charges and is completed promptly to meet payer requirements.
  • Includes necessary details in documentation to reflect the diagnosis and condition accurately (e.g., objective functional measures, special tests, minutes).
  • Signs and cosigns documentation as required by practice standards and policies.
Professional Growth
  • Maintains professional memberships and is encouraged to engage in organizational activities and meetings.
  • After attending continuing education, presents in-services to enhance staff knowledge and skills, improve patient management, and promote responsible practices.
  • Acts as a clinical resource related to rehabilitation therapy for other staff within the organization and community.
  • Stays informed about clinical practices through literature reviews and provides supporting reference materials when presenting in-services or case studies.

  • Maintains licensure/certification and registration in good standing throughout the fiscal year.
  • Direct patient care providers must maintain current BCLS (CPR) and other required certifications.
  • Consistently employs appropriate universal precautions, protective equipment, and ergonomic techniques to safeguard patients and self.
  • Adheres to regulatory agency requirements and compliance processes.
  • Follows established organizational and departmental policies.
  • Available to work overtime and additional shifts as required.

Education and Experience
  • The knowledge, skills, and abilities required are typically acquired through the successful completion of a bachelor's degree. A Physical Therapist license in the State of Indiana is mandatory. A valid state-issued Driver's License is required. Previous experience in acute medical care is advantageous.
Knowledge & Skills
  • Familiarity with modalities and equipment necessary for standard treatments is required.
  • Requires tact and discretion when interacting with patients, families, and physicians.
  • Maintains current BLS certification.
Working Conditions
  • At risk for occupational exposure to blood-borne pathogens.
Physical Demands
  • Requires physical ability and stamina to perform essential functions, including prolonged standing/walking, pushing carts/wheelchairs up to 50 pounds, and lifting patients (maximum of 35 pounds unassisted; over 35 pounds requires assistance).
  • Must possess sufficient balance and strength to ensure patient safety.
  • Physical ability to perform activities such as manual muscle testing, passive range of motion, and manual techniques is required.
