Reports to the Manager. Responsible for assessing patients, devising treatment strategies, and executing physical therapy techniques aimed at restoring functionality, preventing disabilities, alleviating pain, and enhancing overall quality of life following medical conditions, injuries, or surgical procedures.

Clinical Responsibilities
  • Conducts thorough assessments based on physician directives and diagnoses.
  • Creates individualized treatment plans with time-sensitive functional objectives.
  • Regularly reassesses patients and adjusts treatment plans according to objective findings.
  • Communicates directly with physicians regarding any necessary modifications to prescribed therapies.
  • Ensures prescriptions are up-to-date.
  • Supervises therapy assistants in patient treatment planning.
  • Adheres to all regulatory guidelines regarding assistant supervision and patient management.
  • Initiates discharge planning at the evaluation stage.
  • Incorporates skills gained from continuing education into daily practice.
  • Employs a variety of evidence-based treatment methods tailored to individual patient needs to optimize outcomes.
  • Educates patients and families on therapeutic activities, provides relevant materials, and documents in medical records.
  • Maintains professional and appropriate communication with patients and families based on their understanding and needs.
Operational Duties
  • Meets productivity expectations for assigned responsibilities.
  • Actively seeks to assist colleagues when personal caseload is below productivity standards.
  • Demonstrates initiative and adaptability in managing daily caseloads.
  • Takes responsibility for daily support tasks when necessary personnel are unavailable.
  • Participates constructively in meetings, program development, and special projects.
  • Meets all required deadlines (license, CPR, mandatory training, TB testing).
  • Accountable for all information related to policies and regulatory guidelines discussed in meetings and training sessions.
  • Consistently follows team communication norms.
  • Clearly communicates expectations to support staff.
  • Collaborates effectively with all disciplines and ancillary personnel to manage patient care.
Documentation Standards
  • Adheres to departmental policies regarding timely completion of all documentation (evaluations, notes, discharge summaries).
  • Completes all documentation related to patient assessment tools within established guidelines for outcomes and regulatory compliance.
  • Ensures documentation supports all billing charges and is completed promptly to meet payer requirements for prior authorization and treatment continuation.
  • Includes appropriate detail in documentation to support diagnosis and condition.
  • Signs and cosigns documentation as required by practice regulations and policies.
Professional Growth
  • Maintains professional memberships and is encouraged to engage in organizational activities and meetings.
  • After attending continuing education, presents in-services to enhance staff knowledge and skills, improve patient management, and promote responsible patient care.
  • Acts as a clinical resource for rehabilitation therapy for other staff within the organization and community.
  • Stays informed of clinical practices through literature reviews and provides reference materials when presenting in-services or case studies.
Contributing to Department Effectiveness
  • Completes additional job-related duties and projects as assigned.

  • Direct patient care providers must maintain current BCLS (CPR) and other certifications as required by their position/department.
  • Consistently utilize appropriate universal precautions, protective equipment, and ergonomic techniques to safeguard both patients and self.
  • Adhere to regulatory agency requirements, survey processes, and compliance standards.
  • Follow established organizational and departmental policies.
  • Be available to work overtime and additional shifts as required.

Education and Experience
  • Knowledge, skills, and abilities are typically acquired through the successful completion of a bachelor's degree. Physical Therapist licensure in the State of Indiana is required. A valid State-issued Driver's License is also required. Previous experience in acute medical care is advantageous.
Knowledge & Skills
  • Familiarity with modalities and equipment necessary for standard treatments is required.
  • Requires tact and discretion when interacting with patients, families, and physicians.
  • Maintains current BLS certification.
Working Conditions
  • At risk for occupational exposure to blood-borne pathogens.
Physical Demands
  • Requires physical ability and stamina to perform essential functions, including prolonged standing/walking, pushing carts/wheelchairs up to 50 pounds, and lifting patients (maximum of 35 pounds unassisted; over 35 pounds requires assistance).
  • Must possess sufficient balance and strength to ensure patient safety.
  • Physical ability to perform activities such as manual muscle testing, passive range of motion, and manual techniques is necessary.
